Male Subfertility

Subfertility refers to a situation where a couple are not able to conceive a child even though they've had frequent, unprotected sexual intercourse for at least a year. In about half of these couples, male infertility plays a role. In about 40% of cases, both partners may have health conditions that affect their fertility.

Male subfertility is due to low sperm production, abnormal sperms, or blockages that prevent the delivery of sperm. Illnesses, injuries, chronic health problems, lifestyle choices and other factors can also play a part.

Causes

Male fertility may be affected by medical, environmental or lifestyle causes.

Medical causes:

  • Infection.
  • Varicocele.
  • Ejaculation issues.
  • Cancers and cancer treatment.
  • Undescended testicles.
  • Hormone imbalances.
  • Sperm duct defects.
  • Chromosome defects.
  • Problems with sexual intercourse.
  • Certain medications.

Environmental causes:

  • Industrial chemicals and heavy metal exposure.
  • Radiation or X-rays.
  • Overheating the testicles.

Health, lifestyle and other causes:

  • Urge to urinate
  • Bladder fullness due to sterile solution infusion. You can try to relax during the scope by taking slow, deep breaths.

Tests and diagnosis:

The initial assessment may involve:

  • Substance abuse.
  • Alcohol use and liver disease.
  • Occupation.
  • Smoking.
  • Emotional stress.
  • Obesity.

Treatment

Often, an exact cause of infertility cannot be determined. Even if an exact cause isn't clear, your doctor may be able to recommend treatments that work. In all cases of infertility, the female partner also will need to be checked and may need treatment. In some cases, treatment of the female partner may help compensate for male fertility problems.

Treatments for male infertility include:

  • General physical examination and medical history.
  • Semen analysis.
  • Have chills or fever of more than 38°C
  • Scrotal ultrasound.
  • Transrectal ultrasound.
  • Hormone testing.
  • Genetic tests.
  • Testicular biopsy.
  • Specialized sperm tests.

When treatment doesn't work:

Sometimes male fertility problems can't be treated, and it's impossible for a man to father a child. Consideration for either using sperm from a donor or adopting a child may then be needed.
